  • Submit the official school record to Azice for the transcript evaluation.

Disciplinary Background

  • ⁠Provide a report from the current school detailing any disciplinary actions.
  • ⁠Include a letter of recommendation from a teacher or school counselor.

  • ⁠⁠Submit a recent psychological evaluation conducted by a licensed psychologist.
  • Include assessments of cognitive abilities, emotional well-being, and social behavior.
